| Resumo: | Introdução: A hemostasia é definida como um processo responsável por manter o equilíbrio entre a coagulação e a fluidez do sangue. Alterações nesse sistema têm sido associadas tanto à formação de trombos quanto à ocorrência de hemorragias. Diversos fatores contribuem para esse desequilíbrio, sendo os fatores genéticos considerados os mais relevantes. Objetivo: Investigar variantes genéticas nos genes Fator V Leiden (F5), Protrombina (F2), SERPINA1 (PAI-1), THBS1 (Trombospondina-1) e Metilenotetrahidrofolato Redutase (MTHFR) por meio de um estudo familial. Materiais e Métodos: O presente estudo adotou um modelo descritivo e molecular de estudo familial, envolvendo 22 participantes pertencentes a uma família com histórico de trombose. Foram coletados 5 mL de sangue venoso em tubos contendo EDTA para posterior realização de análises hematológicas e moleculares. A genotipagem das variantes foi realizada por reação em cadeia da polimerase em tempo real (qPCR), utilizando sondas TaqMan. Os resultados foram analisados por meio dos softwares SPSS e GraphPad Prism. Para a comparação das médias dos parâmetros hematológicos entre os diferentes genótipos das variantes, foi utilizada análise de variância de uma via (ANOVA), adotando-se nível de significância de p < 0,05. Resultados: As análises demonstraram ausência de mutações nos genes F5 e F2 em todos os participantes. Apenas um indivíduo apresentou heterozigose no gene SERPINE1. Em relação ao gene MTHFR, 45,4% dos participantes apresentaram genótipo heterozigoto (CT) e 18,1% genótipo homozigoto (TT). Para o gene THBS1, 40,9% eram heterozigotos (AG) e 22,7% homozigotos (GG). Foram observadas associações estatisticamente significativas entre alguns genótipos e parâmetros hematológicos, como concentração de hemoglobina corpuscular média (CHCM), amplitude de distribuição dos eritrócitos (RDW) e hematócrito. Conclusão: O estudo demonstrou a ausência das principais mutações clássicas associadas à trombofilia hereditária na família analisada. As variantes identificadas nos genes MTHFR e THBS1 apresentaram possível influência sobre parâmetros hematológicos, porém sem associação direta comprovada com o risco trombótico. Esses achados reforçam o caráter multifatorial da trombose, envolvendo a interação entre fatores genéticos, metabólicos e ambientais. |
| Abstract: | Introduction: Hemostasis is defined as a process that is responsible for maintaining the balance between coagulation and blood fluidity. Alterations in this system have been demonstrated to favour both the formation of thrombi and the occurrence of haemorrhages. A multitude of factors have been identified as contributing to this imbalance, with genetic factors being the most significant. Objective: To investigate genetic variants in the Factor V Leiden (F5), Prothrombin (F2), SERPINE1 (PAI-1), THBS1 (Thrombospondin-1), and Methylenetetrahydrofolate reductase (MTHFR) genes through a family study. Materials and Methods: The present study adopted a descriptive and molecular family study model, with 22 participants from a family with a history of thrombosis. Five millilitres of venous blood were collected in EDTA-containing tubes for subsequent hematological and molecular analyses. The genotyping of variants was performed by real-time PCR (qPCR) using TaqMan probes. The results were analysed using SPSS and GraphPad Prism software. A one-way analysis of variance (ANOVA) was employed to compare the means of the haematological data among the variant genotypes, with a significance level of p < 0.05 being applied. Results: The analyses demonstrated the absence of mutations in the F5 and F2 genes in all participants. Only one individual presented heterozygosity in the SERPINE1 gene. Regarding the MTHFR gene, 45.4% presented a heterozygous genotype (CT) and 18.1% a homozygous genotype (TT). For the THBS1 gene, 40.9% were heterozygous (AG) and 22.7% homozygous (GG). Statistically significant associations were observed between some genotypes and hematological parameters, such as MCHC, RDW, and hematocrit. Conclusion: The study demonstrated the absence of the main classic mutations associated with hereditary thrombophilia in the family analyzed. The variants found in MTHFR and THBS1 showed a possible influence on hematological parameters, but without a proven direct association with thrombotic risk. These findings reinforce that thrombosis has a multifactorial character, involving interaction between genetic, metabolic, and environmental factors. |
